We need a completed ACH contribution form and a confirmation of your account information. You can attach either a voided check or an account verification letter from your bank. Please note, a bank statement will not suffice, as it does not have your bank’s routing number. The confirmation must have your full name (which must also match the name on your account with us), account number, and routing number.
