Is there any supplemental information needed? (i.e. a statement)

If we do not already have one on file, we generally require an account statement from your existing custodian. We use this information to ensure the accuracy of your transfer request. The statement must contain your full name, account type, and existing custodian. Exceptions can be made if your custodian provides statements that are missing some information, but please be advised this may cause delays in transferring your funds.
