Your name must be on the verification document

The address provided on the application must be listed on the verification document

The document must be dated within 90 days of the request for address verification

The document must either be scanned or a picture (screenshots will not be accepted)

If the document is a piece of mail it must be opened; unopened mail or mail without a date will not be accepted
